Tumescent Liposuction

Tumescent liposuction is a treatment without general narcosis for removal of excess adipose (fatty) tissue from under the skin. This is a very gentle method toward the surrounding tissue and no hospitalization is required. Tumescent liposuction is a modern technique of withdrawing accumulated fat from those parts that are not affected even by physical activity. Certain regions of the body contain lipid reserves that can be defined genetically and no diet or physical exercise will remove them. In such regions after liposuction fat does not reaccumulate so that the result is permanent.

The principle of tumescent liposuction consists in filling the tissue to be withdrawn with a special solution containing an anesthetic which fully desensitizes the given area. The solution is left to act for about 30 minutes when the fatty tissue softens. Then several very thin (2–4 mm) incisions are made and a suction cannula is inserted (the cannula diameter for this type of liposuction is less than 4 mm) and the introduced fluid together with the adipose tissue are withdrawn.

The total treatment is painless since the solution contains a very dilute local anesthetic. It also contains a low concentration of epinephrine which constricts the blood vessels in the region and thus decreases bleeding. Throughout the treatment the client is fully conscious and in good psychic condition, he or she can phone, read or watch the TV.

An advantage of the method lies in the possibility of leaving the client in an upright position and carry out any modifications that would not be clearly visible when reclining on a bed.

During classical liposuction (such as is not carried out here), such as is done in full anesthesia, cannulas up to 1 cm in diameter are used that may affect the subcutaneous tissue, cause large blood contusions and the healing is lengthy and painful. It can even bring about subcutaneous scars which result in visible unevenness of the surface and in scars. Hence this method is on the decline and tumescent liposuction is making huge progress.
Ideal candidates for tumescent liposuction are persons who are not obese but exhibit an overweight of 5–15 kg, are healthy, undergo no drastic diet and join in occasional exercise. There is no age limit for the treatment, it depends entirely on the health and physical condition of the client. With clients over about 55 years of age one can expect that after the removal of the adipose tissue the entire body section will be decreased, skin will attach itself properly but the skin elasticity will not reach 100 %. The client is informed about this completely natural consequence.

Before Treatment

After consultation the client is presented with a list of medicines, the use of which should be avoided before the treatment. Some should be left out already a week before the treatment. These medicines include Ibuprofen, Brufen, Aspirin and those containing the Ginkgo biloba extract. These substances could lead to greater bleeding and formation of bruises. On the treatment day the client should not drink caffeine containing drinks and should not smoke. Alcohol and smoking should be stopped a week before the treatment. On the day of treatment the client should shower since for 24 hours after the treatment he must not remove the compression underwear. It is advisable to secure a sufficient amount of absorbent diapers. The tumescent fluid runs out of the incisions for up to three days so that the bed linen and mattress could be soiled. On the day of treatment the client should avoid consumption of heavy food; readily digestible food in rational amounts is recommended. A large amount of liquid (up to 3 liters a day) is to be drunk. The client should take care of his return home as after the treatment driving a motor vehicle should be avoided.

Postoperative Course

Immediately after the treatment the client is provided with compression underwear that should be worn for two weeks uninterrupted (after liposuction of the belly for up to three weeks). The underwear should be left on the body for the first 24 hours to prevent the formation of a haematoma. It is to be removed only for necessary hygiene. For several hours following the treatment the tumescent fluid leaks through the incisions. They are covered with steri-strips and these should be left in place for 3-4 days. The client should obtain a sufficient number of absorbent diapers to hold the effluent fluid. Such diapers can be obtained in the Medilip Centre. Pain after treatment is minimal and no analgesic is generally required. If pain does occur a common analgesic will do. On the day after treatment the client should stay at home but lying in bed is not required. The tumescent liposuction method decreases the risk of bruise formation, the recovery period is shortened and thus the time of work incompetence is maximally reduced. Return to work is generally possible on the third day after treatment. Application of tumescent liposuction makes it possible to remove as much as 3 liters of fat while shaping the body into the ideal form. Compression underwear should be provided in two sets to permit its regular exchange after washing.

Postoperative medicament

Most clients have a slight complication in the form of bruises in the operated area after tumescent liposuction. After years preparations testing on patients who have had a liposuction with us it has minimal impact on improving healing, reducing the incidence of bruises and only traumatized client's wallet. Recent years proved the effect of preparation after each tumescent liposuction. These capsules contains 3 active substances, which are complementary in their effect. Extract of Rescues aculeate and its mechanism restores the flexibility of the vessel wall, accelerating the flow of blood in the veins and lymphatic vessels and prevents stasis of blood in the veins. Hesperidia-ascorbic acid and metylchalcon moderates pathological changes in the vascular wall, reducing the increased capillary permeability and fragility. This preparation clearly reduces bruising and swelling and accelerates healing in all areas after liposuction. Administration of this preparation is combined with drugs acting locally in the form of ointments, creams, gels or sprays, which themselves applied on the clients after liposuction.

Adverse Effects

During the first hours or even days following the treatment light giddiness may appear after changing body position. Therefore, it is recommended to do the first removal of the underwear while lying down. Following the treatment there is a minute risk of occurrence of swelling and unevenness of skin surface. In most cases they disappear after a certain time. The affected areas should be regularly massaged. If these symptoms persist after six months a physician should be consulted and, if necessary, correction is carried out free of charge. After the treatment the client should avoid exposure to direct sunlight even in a solarium. The incision scars could then turn brown. The treated body parts could be more sensitive than the rest of the body for several months.

Consultations

Consultations are done for free. The client is acquainted with the principle of the method of tumescent liposuction, he or she is shown the instruments and is informed about possible risks considering his or her state of health. If the client agrees to undergo the liposuction, the term is agreed on and an advance payment of 5000 Czech crowns is made (either in cash, or transfer money to our bank account). If this is not done, the term of treatment is canceled. Two weeks before the planned operation the client should come to be measured so as to ensure the correct size of the compression underwear that will be prepared. One week before the liposuction the client should undergo a pre-operation examination at the Modřany policlinic or in the surgery of his or her practical physician.

Techniky liposukce

Suchá technika liposukce

Do tukové tkáně není vstřikován roztok, je třeba použít vyššího odsávacího tlaku a z toho důvodu může dojít k poškození tkáně, vzniku nerovností a většímu krvácení. Tato metoda liposukce se provádí v celkové narkóze, vyžaduje pooperační hospitalizaci a dlouhou dobu nošení kompresivního prádla (6 týdnů až 2 měsíce). Jedná se o překonanou metodu liposukce, která byla na většině pracovišť nahrazena technikami vlhkými.

Vlhké techniky liposukce

Do podkožní tukové tkáně je vstřikován roztok, který je směsí fyziologického roztoku, adrenalinu (způsobí stažení cév) a může obsahovat i anestetikum. Napuštění roztokem dovoluje snazší odsátí tuku a omezuje krevní ztráty. Tyto techniky liposukce dělíme: wet technika liposukce – objem použité tekutiny je obvykle menší než množství odsátého tuku super wet technika liposukce – objem tekutiny je zhruba shodný s množstvím odsátého tuku tumescentní technika liposukce - pojmenování vychází z latinského tumescere = otok, opuchlina. Vstřikovanou látkou vytvoříme uměle tuhý otok, krajina je zcela znecitlivěna a cévním stažením téměř bezkrevná. Množství napouštěné tekutiny bývá několikanásobně větší než objem odsávaného tuku. Touto metodou lze liposukci provádět ambulantně, velkou výhodou je, že pacienta můžeme během výkonu polohovat i postavit a zhodnotit tak výsledek. Jedná se o velmi šetrnou metodu, která omezuje klienta pouze minimálně a návrat k normálnímu životu trvá zhruba 2 až 3 dny.

Laserová liposukce

Využívá kanyly, ve které je zabudován laserový paprsek, tento opět zkapalní tukovou buňku a následně je tuk šetrně odsát. U malých oblastí se tuk většinou neodsává a očekává se, že dojde ke vstřebání. Přístroje využívající tuto metodu jsou Smart Lipo a CoolLipo. smart_lipo2_ Coolipo3

Ultrazvuková liposukce – (ultrasonic – assistet liposuction)

Využívá k odsátí tuku speciální kanylu, která vysílá ultrazvukové vlny. Tyto vedou k porušení stěny tukové buňky, tuk se rozlije a odsávání je jednodušší. Negativní u této metody je možnost popálení kůže, pokud je v rukou nezkušeného operatéra. Tato metoda je často zaměňována s ultrazvukovou neinvazivní liposukcí, kdy sonda vysílající ultrazvukové vlny působí na tukové buňky bez porušení kůže. Množství takto odstraněného tuku , pokud se vůbec o odstranění tuku dá hovořit, je malé a výsledky dle sdělení řady klientů jsou neuspokojivé. Vibrační liposukce – ( power – assisted liposuction )

Tato technika využívá napojení liposukční kanyly na elektrický vibrátor, který kmitá kanylou dopředu a dozadu asi 1mm. Vhodné je využít tuto techniku u oblastí s vyšším množstvím vaziva, např. na zádech nebo bocích u mužů. Naše zkušenosti s touto technikou liposukce jsou spíše negativní, usnadňují sice práci operatérovi, zkracují dobu výkonu, ale naopak zhmožďují operovanou tkáň a při výkonu dochází k většímu krvácení. Tuto metodu z těchto důvodů nevyužíváme.

Jste vhodným adeptem na liposukci?

Ideálním kandidátem na liposukci jsou zdraví jedinci s dobrou elasticitou kůže a ohraničeným nahromaděním tuku v určité oblasti. Dolní věková hranice je 18let, horní věková hranice není omezena. Mnoho lidí si myslí, že liposukce je jedním ze způsobů, jak snížit svoji váhu, avšak není tomu tak. Po liposukci dochází pouze ke změně objemu.
